well offcourse you must give user name and password because JSP is on client side it will be good when you ask it for user name and password... well try to keep database work separate from JSP you must invoke servlet and beans for this purpose...... when ever you do database work in jsp its called a weak practice try to do good pratice
SQL Server is an authentication based database management tool that do required some sort of authentication processes... if you are developing single PC application (sql server and application on same system) and you are using odbc connectivity, then it may work without some specific username/password setting it to the windows authentication ... but it is recommended that you must have a registered user at SQL server so that things can work in fine manners .....
